Unable to Rank Up to UVH 4 When Completing “Her Flaming Vision”

Players who complete the Wildcard UVH (Ultimate Vault Hunter) 3 Rank Up mission “Her Flaming Vision” are unable to rank up from UVH 3 to UVH 4 while failing to complete the “Even More Ultimater Vault Hunter” challenge.

Workaround: Join a multiplayer match with other players completing the same mission.
